    APPLICATION FOR THE MEMBERSHIP

    1.    Please complete all parts of the application form. The College will not process any incomplete application.

    2.    The proposers must be paid-up fellows of the College of Surgeons of Hong Kong.

    3.    Please enclose certified copies of the required supporting documents

    4.    A cheque should be made payable to "The College of Surgeons of Hong Kong Limited".



    Ordinary Membership

    Entrance fee HK$6,000 and annual subscription HK$2,000*

    *Annual subscription for Ordinary Member aged over 65 is HK$200 and Retired Ordinary Member HK$200, for Overseas Member is HK$1,000.

    A processing fee HKD $ 210 will be charged for any unsuccessful application, including incomplete application.

    The cheque will be returned to the applicant by post if the application is unsuccessful.


    Fellowship

    Entrance fee HK$8,000 and annual subscription HK$3,000*

    *Annual subscription for Fellow aged over 65 is HK$300 and Retired Fellow HK$300, for Overseas Fellow is HK$1,500.

    Fellows who are currently Ordinary Members of the College will not be required to pay annual subscription if the applications are made within the calendar year.

    A processing fee of HKD 300 will be charged for any unsuccessful application, including incomplete application.

    The cheque will be returned to the applicant by post if the application is unsuccessful.


    Fellowship without Examination

    Entrance fee HK$8,000 and annual subscription HK$3,000*

    *Annual subscription for Fellow aged over 65 is HK$300 and Retired Fellow HK$300, for Overseas Fellow is HK$1,500.

    A processing fee of HKD 600 will be charged for any unsuccessful application, including incomplete application.

    LIST OF DOCUMENTS REQUIRED FOR APPLICATION

    Ordinary Membership

    All applicants must submit the following documents together with the Application Form in order to complete the application process:

    Certified True Copy of:

    › - University Certificate

    › - Medical Registration Ordinance - Annual Practising Certificate

    › - MHKICBSC Examination - Part 3 Result Slip

    › - Basic Surgical Skills Course (Certificate of Attendance)

    › - Clinical Core Competency Course (Certificate of Attendance)

    › - Other relevant examinations/qualifications(if any)


    Original Copy of:

    - Letter of choosing the MCSHK only


    A crossed cheque of HKD 8,000 (Entrance fee HKD 6,000 & Annual Subscription Fee HKD 2,000) made payable to “The College of Surgeons of Hong Kong Limited”.



    Applicants MUST apply for MCSHK within THREE years from the date they were eligible to be conferred. Applications submitted beyond this deadline will NOT be accepted.


    Fellowship

    All applicants must submit the following documents together with the Application Form in order to complete the application process:

    Certified True Copy of:

    › - University Certificate

    › - Medical Registration Ordinance - Annual Practising Certificate

    › - Joint Specialty Fellowship Examination Result Slip / Specialty Fellowship Examination Result Slip(for Plastic Surgery only)

    › - Certificate of Attendance of all mandatory courses required by the respective specialty board

    ›  - Certificate for Clinical Proctorship Programme for Basic and Advanced Laparoscopic Surgery (applicable to Higher Surgical Trainees in General Surgery/Urology)

    ›  - Other relevant examinations/qualifications(if any)

    Original Copy of:

    › - Index Operation Record Form (only applicable to Higher Surgical Trainees in General Surgery admitted from 1 July 2010 onwards)

    A crossed cheque of HKD 11,000 (Entrance fee HKD 8,000 and Annual Subscription Fee HKD 3,000) made payable to “The College of Surgeons of Hong Kong Limited”.


    Fellowship without Examination

    All applicants must submit the following documents together with the Application Form in order to complete the application process:

    Certified True Copy of:

    › - University Certificate

    › - Medical Registration Ordinance - Annual Practising Certificate

    › - Other relevant examinations/qualifications(if any)

    A crossed cheque of HKD 11,000 (Entrance fee HKD 8,000 and Annual Subscription Fee HKD 3,000) made payable to “The College of Surgeons of Hong Kong Limited”.


    TERMINATION/WITHDRAWAL OF MEMBERSHIP

    If any Member shall fail to pay this annual subscription or other sums which may be due from him to the College for a period of three months after the same as become due, notice shall be sent to him calling his attention thereto and if he does not pay the account within one month following he shall be erased from the books of the College. However, if at any time he shall give to the Council a satisfactory explanation he may at the discretion of the Council and upon payment of arrears be re-admitted into membership.


    A Member may resign by notice in writing to the Secretary but he shall be liable for payment of his subscription for the current year in which he resigns.


    If any Member violates the Constitution of the College or if his conduct is such as is, in the opinion of the Council, likely to be injurious to the interests of the College and if the Member complained of shall fail to explain his conduct to the satisfaction of the Council they may declare that such Member shall cease to be a Member of the College and his name shall be removed from the list of Members. Such Member may rejoin if he reapplies via the normal channels.


    When a Member ceases to be a Member of the College either by resignation or otherwise, the College shall not return any subscription paid by such Member before he ceased to be a Member.


    Note:

    All Members have responsibility to inform the College for any change of particulars e.g. corresponding address, e-mail address and place of work etc.
    The College will not be responsible for any issues arise as a result of your failure to inform the College.
